The 2024-25 Online Verification/Update (OVU) packet is available in ParentVUE.The OVU packet allows parents to electronically update phone numbers, emergency contacts, health conditions, and add new immunizations dates for their students via ParentVUE.

Required Forms 

1. Emergency and Health Care Forms
   • All students must have accurate emergency and health care information each school year. The Online Verification Update packet or OVU will allow parents to review and update phone numbers, emergency contacts and health information (such as health conditions and immunizations) for students in grades PS/HS  and kindergarten through 12th grade. Enrolling parents will be able to access OVU through their SIS ParentVUE account.
2. Digital Resource Consent- Teachers will use a variety of digital resources to help students learn during the 2024-25 school year. These digital resources have been checked and approved by Fairfax County Public Schools. Please provide your consent for your student to use these resources through the Parent Digital Consent System
3. After School Program Registration 
   • Parents need to complete the online Google registration form to give permission for your child to participate in the After School Program at Frost Middle School.  Beginning September 11th, the Frost After School Program will begin meeting on Monday and Wednesdays only. The full 5 day program will begin on October 2nd from 2:30 - 4:30pm daily.  Late buses will be provided on Monday, Tuesday, Wednesday & Thursday.  Parent pick up will be on Friday.
4. SOL Retake Parent Permission Form
   • The Virginia Board of Education allows middle school students who fail Standards of Learning (SOL) tests by a narrow margin to retake the test before the end of the school year. Retesting is available when a student fails their first attempt with a score of 375-399 (where 400 is passing). Parent permission is required for any retests in grades 3-8 and End of Course math (Algebra 1, Geometry & Algebra 2). If you believe that your student would benefit from a second opportunity to demonstrate their knowledge and skills on the SOL test, please complete this permission form (coming soon) to let us know if you want your child to retest or not.  A decision not to allow retesting will not impact your student’s course grade or academic record. Please complete the form to let us know if you want them to retake the test OR if you do not want them to retest.
5. Student Rights and Responsibilities (SR&R) Parent Signature Sheet- Please log on to your SIS ParentVue account and sign to acknowledge review of this document. For information and to view the SR&R document visit the FCPS SR&R page. 
6. Go Paperless for report card distriubtion- Under Report Card Tabs choose "Go Paperless" in SIS ParentVUE
7. Intent to Ride FCPS School Bus- information found in SIS ParentVUE

If you are unable to complete forms online, we will have paper copies on hand and be collecting these important beginning-of-the-school-year forms on these dates.  Location TBD.
